Facility Reporting Form
In order to effectively keep our facilities running successfully and efficiently, it is crucial that we are aware of any situations when things are not functioning properly. This report, which is an important part of our operations, is due at the end of every month. Once it is filled out completely and accurately, we will take the necessary steps to implement the required improvements.
Additionally, please keep in mind that it is your responsibility on a daily basis to maintain the building in good condition, which includes tasks such as but are not limited to daily trash removal, sweeping floors, ensuring that paper towels and toilet paper are replaced as needed and water coolers or drinks replenished.
